Intake and Scheduling

Through one call, parents and providers can begin the intake and scheduling process with many of the six partner organizations located at CDS.  Furthermore, most parents only have to complete one set of intake paperwork for the different providers from which they might receive services.  For more information, contact us by phone at 864.331.1302, by fax at 864.331.1419, or simply complete the fields on our Intake and Scheduling Form, and one of our specialists will be in touch with you.

Medical Records

CDS handles the medical records of Developmental-Behavioral Pediatrics and Kidnetics of the Children's Hospital, Greenville Hospital System; and Clarity: the Speech, Hearing & Learning Center.  In this way, only one chart exists on a patient who uses more than one of these organizations, providing for improved information-sharing on a case.  CDS's Notice of Privacy Practices explains our commitment to keeping your child's health information private and secure and your rights as a patient representative to access this information.  For more information, contact us by phone at 864.331.1306 or by fax at 864.331.1319.

Spanish Support

CDS has a full-time Spanish Support Assistant on staff to help Spanish-speaking families access and understand the services available through the CDS Partnership. This staff person assists with Intake, Scheduling, and translation services during evaluations and appointments. We have materials available in Spanish and continually seek representation in the community at large to reach this growing population. For more information, contact us by phone at 864.331.1445.

Media Resources

CDS has wonderful resources for children, parents and professionals to use at the center or to check out.  For waiting parents and siblings, CDS has a free Wi-Fi network for visitors to use. We also have a Spanish-English information kiosk in our main lobby as well as a reading area stocked with children’s books.

Additional materials including information for parents and caregivers about developmental issues and coping, professional materials about specific conditions and treatments, and a variety of children's books and DVDs may be found in the upstairs media center. The media center is equipped with a flat screen TV and DVD player, three computers with internet access, as well as a Little Tykes learning computer. This area is maintained by CDS volunteers and is open during regular business hours.
